Mechanism

Aripiprazole acts as a partial agonist at dopamine D2 and serotonin 5‑HT1A receptors, and as an antagonist at serotonin 5‑HT2A receptors. This unique mechanism stabilizes dopamine and serotonin activity, reducing psychotic symptoms while minimizing extrapyramidal side effects.

Pharmacokinetics

  • Absorption: Oral bioavailability ~87%; peak plasma levels in 3–5 hrs.
  • Distribution: Widely distributed; ~99% plasma protein bound.
  • Metabolism: Hepatic via CYP2D6 & CYP3A4; active metabolite dehydro-aripiprazole contributes ~40% exposure.
  • Excretion: Excreted via urine (~25%) and feces (~55%).
  • Half-life: Aripiprazole ~75 hrs; metabolite ~94 hrs.

Side Effects

  • Headache
  • Insomnia
  • Anxiety
  • Nausea, vomiting
  • Weight gain, metabolic changes
  • Rare: Tardive dyskinesia, neuroleptic malignant syndrome

Contraindication

  • Hypersensitivity to Aripiprazole
  • Elderly patients with dementia-related psychosis (↑ mortality risk)

Interaction

  • CYP3A4 inhibitors (e.g., ketoconazole) ↑ Aripiprazole levels
  • CYP3A4 inducers (e.g., carbamazepine) ↓ Aripiprazole levels
  • CYP2D6 inhibitors (e.g., fluoxetine, paroxetine) ↑ Aripiprazole levels

Warning and Precautions

  • Boxed warning: ↑ risk of suicidal thoughts in children, adolescents, and young adults
  • Monitor metabolic parameters (weight, glucose, lipids)
  • Use cautiously in patients with cardiovascular disease
