Amlodipine

Tablet

Amlodipine is a long-acting calcium channel blocker used in the management of hypertension and angina. It lowers blood pressure by relaxing vascular smooth muscle and improves oxygen supply to the heart. It is widely prescribed as first-line therapy for cardiovascular protection.

Mechanism

Amlodipine inhibits calcium influx into vascular smooth muscle and cardiac muscle. This causes vasodilation, reduces peripheral resistance, lowers blood pressure, and decreases myocardial oxygen demand.

Pharmacokinetics

  • Absorption: Well absorbed orally; bioavailability ~60–65%.
  • Distribution: Widely distributed; protein binding ~93%.
  • Metabolism: Hepatic metabolism via CYP3A4.
  • Elimination: Excreted in urine and feces.
  • Half-life: ~30–50 hours (long-acting).

Side Effects

  • Peripheral edema
  • Headache
  • Dizziness
  • Flushing
  • Fatigue

Contraindication

  • Hypersensitivity to Amlodipine
  • Severe hypotension
  • Cardiogenic shock
  • Severe aortic stenosis

Interaction

  • CYP3A4 inhibitors (increase plasma levels)
  • Other antihypertensives (additive effect)
  • NSAIDs (may reduce antihypertensive effect)

Warning and Precautions

  • Use under medical supervision only
  • Monitor blood pressure regularly
  • Caution in hepatic impairment
  • Consult physician during pregnancy and lactation
