Alfuzosin

Tablet

Alfuzosin is an alpha-1 adrenergic receptor blocker used in the management of benign prostatic hyperplasia (BPH). It relaxes smooth muscle in the prostate and bladder neck, improving urinary flow and reducing symptoms of obstruction. Alfuzosin is prescribed to relieve lower urinary tract symptoms associated with BPH.

Mechanism

Alfuzosin selectively blocks alpha-1 adrenergic receptors in the prostate and bladder neck. This reduces smooth muscle tone, decreases resistance to urinary flow, and improves bladder emptying in patients with BPH.

Pharmacokinetics

  • Absorption: Oral absorption enhanced with food
  • Distribution: Highly protein bound
  • Metabolism: Hepatic metabolism via CYP3A4
  • Elimination: Fecal and renal excretion
  • Half-life: ≈ 10 hours (extended-release formulation)

Side Effects

  • Dizziness
  • Headache
  • Fatigue
  • Postural hypotension
  • Rare: syncope

Contraindication

  • Severe hepatic impairment
  • Hypersensitivity to Alfuzosin

Interaction

  • CYP3A4 inhibitors (ketoconazole, ritonavir) — increase plasma levels
  • Antihypertensives — additive hypotensive effect
  • Nitrates — risk of severe hypotension
  • Other alpha-blockers — enhanced risk of orthostatic hypotension

Warning and Precautions

  • Monitor blood pressure regularly
  • Use cautiously in elderly patients
  • Advise patients to avoid sudden posture changes (risk of dizziness)
