Mechanism

Alendronate binds strongly to hydroxyapatite in bone and inhibits osteoclast-mediated bone resorption. It interferes with the mevalonate pathway, leading to osteoclast apoptosis and reduced bone turnover.

Pharmacokinetics

  • Absorption: Poor oral bioavailability (~0.6%)
  • Distribution: Rapidly taken up by bone
  • Metabolism: Not metabolized
  • Excretion: Renal excretion unchanged
  • Half-life: >10 years in bone

Side Effects

  • Esophageal irritation, dyspepsia, abdominal pain
  • Musculoskeletal pain
  • Hypocalcemia
  • Rare: Osteonecrosis of the jaw, atypical femoral fractures

  • Esophageal abnormalities (e.g., stricture, achalasia)
  • Inability to stand or sit upright for 30 minutes
  • Hypocalcemia
  • Severe renal impairment (CrCl <35 mL/min)

Interaction

  • Calcium supplements, antacids, iron: Reduce absorption if taken together.
  • NSAIDs: Increase risk of GI irritation.
  • Aminoglycosides: May increase risk of hypocalcemia.

Warning and Precautions

  • Ensure adequate calcium and vitamin D intake.
  • Monitor renal function and serum calcium.
  • Discontinue if severe bone, joint, or muscle pain occurs.
