Mechanism

Acarbose inhibits intestinal alpha-glucosidase enzymes, delaying carbohydrate breakdown and absorption. Metformin decreases hepatic glucose production, enhances insulin sensitivity, and improves peripheral glucose uptake.

Pharmacokinetics

  • Absorption: Acarbose minimally absorbed; Metformin absorbed in small intestine.
  • Metabolism: Acarbose metabolized by intestinal bacteria; Metformin not metabolized significantly.
  • Excretion: Acarbose via feces; Metformin excreted unchanged via urine.

Side Effects

  • Flatulence and abdominal discomfort (Acarbose)
  • Diarrhea and nausea (Metformin)
  • Rare: Lactic acidosis (Metformin)

Contraindication

  • Severe renal impairment
  • Hepatic dysfunction
  • Gastrointestinal disorders (for Acarbose)
  • Hypersensitivity to components

Interaction

  • Alcohol (increases risk of lactic acidosis with Metformin)
  • Insulin or sulfonylureas (risk of hypoglycemia)
  • Antibiotics (may alter Acarbose efficacy)
  • Cationic drugs (reduce Metformin clearance)

Warning and Precautions

  • Monitor renal function regularly
  • Discontinue before radiological procedures with contrast agents
  • Use cautiously in elderly patients

Dosage

As directed by the Physician.
