Teneligliptin

Tablet

Teneligliptin is a dipeptidyl peptidase‑4 (DPP‑4) inhibitor that enhances incretin activity, increasing insulin secretion and decreasing glucagon release. It is prescribed for type 2 diabetes mellitus to improve glycemic control.

Mechanism

Teneligliptin inhibits DPP‑4 enzyme, prolonging the activity of incretin hormones (GLP‑1 and GIP). This increases insulin secretion in a glucose‑dependent manner and reduces glucagon release, improving glycemic control.

Pharmacokinetics

  • Absorption: Rapidly absorbed after oral administration; peak plasma concentration reached in 1–2 hours.
  • Bioavailability: High oral bioavailability (~85%).
  • Distribution: Moderately protein bound (~60–70%).
  • Metabolism: Metabolized primarily in the liver via CYP3A4 and partially excreted unchanged.
  • Excretion: Eliminated via both renal (~34%) and hepatic (~66%) routes.
  • Half-life: Terminal elimination half-life ~20–24 hours, supporting once‑daily dosing.

Side Effects

  • Hypoglycemia (rare, usually with sulfonylureas/insulin)
  • Headache
  • Nasopharyngitis
  • Gastrointestinal upset

Contraindication

  • Hypersensitivity to Teneligliptin
  • Severe renal impairment
  • Severe hepatic impairment

Interaction

  • CYP3A4 inhibitors (ketoconazole may increase plasma levels)
  • CYP3A4 inducers (rifampicin may reduce efficacy)
  • Other antidiabetic agents (risk of hypoglycemia when combined)

Warning and Precautions

  • Use under medical supervision only
  • Monitor renal and hepatic function during therapy
  • Adjust dose in elderly patients

Dosage

As directed by the Physician.
