Mechanism

Ranolazine is an anti-anginal medication primarily used for the treatment of chronic stable angina. It exerts its therapeutic effect by selectively inhibiting the late inward sodium current (late INa) in cardiac myocytes.

Pharmacokinetics

  • Absorption: Oral bioavailability ~35–50%; peak plasma levels in 2–5 hrs.
  • Distribution: ~62% protein bound.
  • Metabolism: Extensively metabolized hepatically via CYP3A and CYP2D6.
  • Excretion: Renal and fecal excretion.
  • Half-life: ~7 hrs.

Side Effects

  • Dizziness
  • Constipation
  • Nausea
  • Headache
  • Rare: QT prolongation

Contraindication

  • Severe hepatic impairment
  • Concomitant use with strong CYP3A inhibitors (e.g., ketoconazole)
  • Hypersensitivity to Ranolazine

Interaction

  • CYP3A inhibitors ↑ ranolazine levels
  • CYP3A inducers ↓ ranolazine efficacy
  • Other QT-prolonging drugs ↑ arrhythmia risk

Warning and Precautions

  • Monitor ECG for QT prolongation
  • Use cautiously in renal impairment
  • Adjust dose in elderly patients

Dosage

As directed by the Physician.
