Pregabalin

Capsule

Pregabalin is a structural analogue of the neurotransmitter gamma-aminobutyric acid (GABA), although it does not act directly on GABA receptors. It binds with high affinity to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system. By reducing calcium-dependent release of several excitatory neurotransmitters, pregabalin decreases abnormal neuronal excitability and can reduce the transmission of pain signals. It is used clinically in the management of specified neuropathic pain conditions and as adjunctive therapy for certain partial-onset seizures.

Mechanism

Pregabalin binds with high affinity to the α2δ subunit of voltage-gated calcium channels, particularly those present on presynaptic nerve terminals.
When a nerve cell is activated, voltage-gated calcium channels normally open and allow calcium ions to enter the presynaptic terminal. Pregabalin reduces the functional activity of these channels, thereby reducing calcium entry.
The reduction in presynaptic calcium influx decreases the release of several excitatory neurotransmitters, including glutamate, noradrenaline and substance P.
Lower release of excitatory neurotransmitters reduces excessive neuronal signalling and hyperexcitability within the central nervous system.
In neuropathic pain, abnormal sensory neurons can become excessively excitable and transmit amplified pain signals. By reducing excitatory neurotransmitter release, pregabalin modulates abnormal pain signalling, thereby reducing neuropathic pain.
Excessive neuronal excitation also contributes to seizure activity. By reducing excitatory neurotransmitter release and neuronal hyperexcitability, pregabalin can help reduce seizure activity when used for its approved seizure indication.

Dosage

  • As directed by the Physician.

Side Effects

  • Fatigue or tiredness
  • Dry mouth
  • Headache

Contraindications

  • Known hypersensitivity to pregabalin or any component of the formulation.

Drug Interactions

  • Pregabalin may have additive effects on the central nervous system when used with other medicines that cause sedation or respiratory depression.
  • Patients should inform their healthcare professional about all prescription medicines, OTC medicines and other substances they are taking before starting pregabalin.
  • Particular caution is warranted with other CNS depressants, because combined use can increase sedation and respiratory-depression risk.
