Minoxidil

Tablet

Minoxidil is a potent direct-acting vasodilator used in the treatment of severe or refractory hypertension. It works by opening potassium channels in vascular smooth muscle, leading to hyperpolarization and relaxation of arterioles. Due to its strong effect, Minoxidil is prescribed with beta-blockers and diuretics to counter reflex tachycardia and fluid retention.

Mechanism

Minoxidil activates ATP-sensitive potassium channels in vascular smooth muscle. This causes hyperpolarization, relaxation of arterioles, and a marked reduction in peripheral vascular resistance, lowering blood pressure.

Pharmacokinetics

  • Absorption: Rapid oral absorption
  • Distribution: Widely distributed in tissues
  • Metabolism: Hepatic metabolism to active sulfate metabolite
  • Elimination: Renal excretion
  • Half-life: ≈ 4 hours

Side Effects

  • Reflex tachycardia
  • Fluid retention and edema
  • Hypertrichosis (excess hair growth)
  • Headache
  • Pericardial effusion (rare)

Contraindication

  • Pheochromocytoma
  • Hypersensitivity to Minoxidil

Interaction

  • Beta-blockers (used to prevent reflex tachycardia)
  • Diuretics (used to prevent fluid retention)
  • Other antihypertensives (additive blood pressure lowering)
  • NSAIDs (may reduce antihypertensive effect)

Warning and Precautions

  • Always co-administer with beta-blocker and diuretic
  • Monitor blood pressure and fluid balance regularly
  • Use cautiously in patients with cardiac disease
