Mechanism

Levonorgestrel prevents pregnancy primarily by inhibiting or delaying ovulation. It also thickens cervical mucus, making it difficult for sperm to reach the egg, and alters the endometrial lining to reduce chances of implantation.

Effectiveness

  • Within 24 hours: ≈95% effective
  • 25–48 hours: ≈85% effective
  • 49–72 hours: ≈58% effective

Pharmacokinetics

  • Absorption: Rapid oral absorption
  • Distribution: Widely distributed, binds to plasma proteins
  • Metabolism: Hepatic metabolism via CYP3A4
  • Elimination: Renal and fecal excretion
  • Half-life: 24–30 hours

Side Effects

  • Nausea and vomiting
  • Abdominal pain
  • Dizziness
  • Menstrual irregularities
  • Breast tenderness

Contraindication

  • Known or suspected pregnancy
  • Hypersensitivity to Levonorgestrel

Interaction

  • Enzyme inducers: Rifampicin, phenytoin, carbamazepine, barbiturates — may reduce effectiveness
  • Herbal products: St. John’s Wort — decreases plasma levels
  • Antiretrovirals: Ritonavir, efavirenz — may lower contraceptive efficacy
  • Other hormonal contraceptives: Potential additive effects

Warning and Precautions

  • Not intended for regular contraception
  • Menstrual delay >1 week requires pregnancy test
  • Use cautiously in patients with liver disease
