Cefpodoxime + Ofloxacin

Tablet

Cefpodoxime + Ofloxacin is a combination antibiotic therapy that provides broad-spectrum coverage against both Gram-positive and Gram-negative bacteria. Cefpodoxime, a third-generation cephalosporin, inhibits bacterial cell wall synthesis, while Ofloxacin, a fluoroquinolone, inhibits bacterial DNA replication. This dual mechanism enhances antibacterial efficacy and reduces the risk of resistance, making it highly effective in treating mixed and severe infections.

Mechanism

Cefpodoxime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death (bactericidal action). Ofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for DNA replication, transcription, and repair. The combination provides a synergistic effect by targeting both the bacterial cell wall and genetic material, resulting in enhanced antibacterial activity and broader spectrum coverage.

Pharmacokinetics

  • Absorption: Both drugs are well absorbed orally
  • Distribution: Wide tissue penetration
  • Metabolism: Minimal hepatic metabolism
  • Elimination: Primarily excreted via kidneys
  • Half-life: Cefpodoxime ~2–3 hrs, Ofloxacin ~5–7 hrs

Side Effects

  • Nausea
  • Diarrhea
  • Abdominal discomfort
  • Dizziness
  • Headache

Contraindication

  • Hypersensitivity to cephalosporins or fluoroquinolones
  • History of tendon disorders with quinolones
  • Epilepsy or CNS disorders

Interaction

  • Antacids (reduce absorption of Ofloxacin)
  • NSAIDs (increase CNS stimulation risk)
  • Warfarin (enhanced anticoagulant effect)

Warning and Precautions

  • Use cautiously in renal impairment
  • Complete full antibiotic course
  • Monitor for allergic reactions
  • Use cautiously in renal impairment
  • Avoid in pregnancy and children unless prescribed
  • Risk of tendonitis/tendon rupture (quinolone class)
